MEMBER’S DATA PRIVACY CONSENT
The Valuecare Member's Data Privacy Consent Form outlines how Valuecare collects, processes, and protects the personal data of its members. IMPORTANT: Please read carefully. In line with Republic Act No. 10173, the Philippine Data Privacy Act of 2012, and its rules, Value Care Health Systems, Inc. (Valucare) uses proper security measures to protect your personal data. Valucare will collect, use, disclose, retain, and transfer your personal information only for relevant purposes, maintaining strict confidentiality as outlined in Valucare’s Data Privacy Policy, which you can access at https://valuecarehealth.com/policy/.
• Data Collected: When providing services to you and your registered dependents, Valucare will process personal data, including your name, age, date of birth, address, nationality, gender, contact information, work, medical history, diagnosis, hospitalization, treatments, prescriptions, laboratory results, health insurance information, and any medical advice related to benefits or claims under the Health Service Agreement with you or your employer/insurance provider.
• Use of Personal Data:We use the personal data we collect to provide healthcare services, including consultations, treatments, and procedures. This data also allows us to process insurance claims, reimbursements, and payments, and to manage member records by updating contact information and dependents. Additionally, we use your data to comply with legal and regulatory requirements. With your consent, we may inform you about our products and services and conduct member satisfaction surveys. Furthermore, we utilize your data for research and development to create new services.
• Data Sharing: Your personal information may be shared with healthcare providers like hospitals, clinics, and physicians involved in your healthcare. We also share your data with insurance companies for claims processing, regulatory authorities as required by law, and third-party service providers who assist in delivering healthcare and administrative services. Additionally, we may share your information with your employer when permitted by law or contract. We also collaborate with our parent company, affiliated entities, both locally and internationally (referred to collectively as "representatives"). We share your personal information with these representatives for legitimate business purposes that benefit you, such as transaction processing, profiling, historical analysis, or fulfilling obligations under agreements with you or your employer.
• Retention, Protection & Disposal: Valucare will retain your personal data as required by law or for as long as necessary to maintain your medical records and comply with applicable regulations. Your next of kin and/or Legal Representative have rights concerning the personal data collected from you and them. We enforce reasonable and appropriate organizational, physical, and technical security measures to safeguard the personal data we collect. These measures are designed to preserve the availability, integrity, and confidentiality of personal data, preventing accidental or unlawful destruction, alteration, disclosure, or any other unauthorized processing.
• Data Subject Rights: You and your authorized representatives are entitled to certain rights in relation to the Personal Information, including the right to access, correction, to object to the processing of the same, and all other related rights provided by law.
